There are some really nice looking ones from Boorum & Pease on Amazon.

The Boorum & Pease stuff looks interesting.  I had not heard of them before your post.  Over 100 year history, but now owned by Tops.  Hardcover, sewn bindings.  From looking at product postings it appears that most are called "record books" and are what I would describe as just a lined notebook, not a multi-column ledger that I assume the OP wants, although they do certainly make those as well.  But if you want a hardcover sewn lined notebook with US legal sized pages (8.5x14) they make it.  Many large sizes we would consider unusual, but would appeal to people that occasionally post here looking for larger notebooks.

 

I have no clue about the paper, and the prices are too high for me to be the guinea pig.

I had a vintage  Boorum and Pease record book and loved it! I didn't realise that they were still around. Are they as good as they used to be?

I haven't tried one recently, but Office Depot has a pretty good return policy.

I have a couple of different sizes of ledger paper. In standard letter size a 4 column, and the paper is pretty good (Made in Brazil) National Brand.

The other is an Officemax brand 13 column 14 w x 8.5. It's from 2005, haven't used it as much.  (China)

 

The 4 column has only 4 or 5 sheets left, as I use it pretty regularly.
