Try changing the way you grasp a pen. This is called 'forefinger up'.

The thumb is flat, the pen rests there only. The forefinger rests with only enough pressure at 12:-12:30-13:00 to keep the pen from doing back flips.

It is an automatic light grip.

My thumb could be a bit flatter.

I also have arthritis so I'm looking into one of these, TWSBI Vac 700, looks like it would fit me well and be easy to control.

I have various hand problems and I write a lot.

 

My Aurora Optima is probably my most comfortable pen, because of the lightness, the width and the long section. I also find the Naka-Ai very comfortable.

 

On the cheaper end of the spectrum is the Noodler's Ahab, also very comfortable, but much less sturdy.

I would second the Rotring Skynn. They are still available in eBay for less than $30, including shipping.

For those occasions you need a roller-ball or ball point, they have matching ones. If you get the ballpoint, replace the refill with a Parker Gel refill for easy writing.

 

As Bo Bo Olson said, check your grip. Also look into changing your writing to use "Shoulder Writing", so that all your fingers are doing is holding the pen, and all movement comes from your shoulder and elbow.

Delta Roma Imperiale is a nice large piston-filler offered in three colors of ebonite and comes with an equally nice large 14k nib. Available for just under $500 from italianpens on ebay. If he doesn't have the size nib you prefer, $25 plus shipping to Yafa (US distributor in LA) will get you what you need in eight weeks or so.

India has some pens that look like the Nakaya Jumbo pens (maybe they were made by Platinum/Nakaya once) take that for a spin

or the Tombow Egg... it's a smaller modern day cousin of the jumbo pens

you may not be looking for oversized pens in terms of length but on girth

My mum is also an arthritis sufferer and asked me the same question about 2 years ago.

 

I showed her my Omas 360 ballpoint and she loved the shape- it's easy for her to grip and she doesn't exert too much pressure on her fingers holding the pen in place- round grips on fountain pens tend to slip and slide a bit.

 

I know the shape of the Omas 360 isn't to everyone's taste, but it's worth a try.
