I am looking for an really large japanese pen. What do you advice?

This will only get me in trouble, but here goes:

Sailor King of Pens
Danitrio Mikado

Two of the biggest I know and both companies have great nibs.

Large in terms of the nib or the actual pen?
I currently am loaning a Sailor Sapporo with a Zoom nib.
Medium sized pen, with a VERY broad & smooth nib.

Also Namiki Emperor.

I second the Namiki emperor.

And the Danitrio Genkai is a large pen.

Since you're asking for extra large, both these are really extra large...

I was looking for some comparison pics but can't find any readily. They really are both HUGE pens... make a MB 149 look impotent..

Hold on - here's one borrowed from Kevin (Genkai on the bottom, MB 149 on top)

Another borrowed from KMPN (Yukari on top, Emperor on bottom)

I guess it depends on how much you want to spend. The "budget" way to go is an Urushi or Raw Ebonite DaniTrio Mikado. RE's are only available on the used market. Great nibs, very usable, if you can deal with the size. Holds a TON of ink, maybe 5ml, IIRR!

The "uber-pen" would be the DaniTrio Genkai, which was available as a RE model.....Maybe you can get a straight Urushi model? Ask Kevin/winedoc.

Sailor KOP's are $500 for the old style with the band (uncommon on the used market), or $900+ for the newer bandless pens. I found the nib too long for easy use, personally, and I sold mine. Roughly the same size as a MB 149.

Namiki Yukari or Emperor pens are big, but very expensive: >1k, in general. The Emperor is huge! Very high quality, but I'm not convinced that they're any better than equivalent Dani products. YMMV.

Ok, A straight Urushi Genkai, how am I going to find one? Kevin/winedoc? Who/what is that?

The Genkai is a great choice. Kevin Chang, aka winedoc, is a great seller and has super customer service. You can find him in the Classifieds as a seller or try <internetpens@gmail.com>.
