I have a beautiful urushi Densho that has only seen my desk and desk drawer in its lifetime. I always keep the ink adjuster (non-nib end of the pen) open to assure ink flow quickly. Question: If I cap the pen and put it in my shirt pocket, and DO NOT close the ink flow adjuster, is there any danger the pen will leak ink from the adjuster area?

I'd suggest closing the adjuster if you are going to carry this pen in your shirt pocket. Not out of any fear of leakage from the adjuster, but in order to minimize any possibility of leakage from the other end.
In truth, I wouldn't carry this pen in my pocket at all. It seems to me it is a pen meant to live on the desk or in the drawer. Too many risks out in the world for beautiful things. But that's just me.

I routinely keep the blind cap opened up, but I carry the pen in my briefcase, not my pocket. Still, I've never had ink leak from the nib. But if I were going to carry it in my pocket, I'd make sure to put it in a pen case of some kind, at the very least to protect the pen, and also just in case of a leak. I did have a slight leak from the barrel once because there wasn't enough vacuum grease on the threads.

Should be fine as long as the nib is pointed up and you are not bungee jumping or playing soccer/contact sports etc. :-)
BUT, even if you close the end knob and decided to write a quick note, there should be enough reserve in the section to write a few lines.
