I'm looking for a pen case that:

Carries 1 Pen
Closes with velcro
Will fit in my pocket
Is inexpensive (around $20)

What do you recommend?

I don't know of any that meet your specifications, but you might want to look into making your own pen case out of cloth or leather. You would then have the flexibility of being able to add velcro to it.

Thats an interesting idea. The velcro isnt my biggest concern, I just thought it would be a nice touch. Do you know where I can buy some leather or cloth? would a craft store have some?

I'm pretty sure it would. You wouldn't need a lot- you might even be able to get by with scrap cloth or leather.

I'm not sure if Velcro is a good idea, it tends to lose its grip over time.

There's a company called Artex or Artec or something...hold on...*checks*...Yep. Artex. They make some very nice pen-cases/pouches which you can buy. I have one made of black leather, for two pens.

I don't like velcro cases; as previously mentioned the velcro looses grip over time. I also dislike the velcro noise.

I found some great leather slip-tab style cases on Ebay, very slim and inexpensive.
