If you are in the U.S., Mont Blanc costs $30, exclusive of your shipping and insurance costs. You send the pen, stamped certificate of authenticity and a cover letter to Mont Blanc. They'll send you a quote in about a week. You either mail or fax it back and they start work. Your pen should come back about a week after they get your form.
When I had mine done, there was no option of keeping my old nib. I guess you might be able to for $400 or so, at least in Asia. MB tries to control the flow of nibs not attached to pens, to minimize counterfeiting.
I don't have any data on nib exchanges from a nibmeister. They would probably re-grind your nib to a smaller size for around $30-60 and a longer waiting period. They might be able to swap the nib assembly for a non MB branded one. The advantage of having a nibmeister do a grind is you'll likely get a nicer writing nib at the end of the day. The downside is, your wait will be longer. Last I looked, Richard Binder's wait time was around 19 weeks.
