I did this myself with a scanner. The process is tedious and can get quite complicated but here are the basics. Write out all the characters you want to include. The handwriting samples are scanned. The characters' silhouettes are cropped and edited in a graphics program then imported as a glyph in the font software. Adjustments are made (spacing, leading, kerning, etc.).

There are websites that will do this for you like Fontifier for $9.
