If the pocket clip is loose, just tighten the jewel. Three-ribbed jewels are typically threaded. The jewel holds the pocket clip in place. This makes cleaning and polishing the cap easier.
You can use a drop of clear finger nail polish to hold the trim ring in place. Be careful because the acetone will soften the cap material.

Your nib unit is likely plugged. Try soaking and removing it. Another possibility is a pin hole in the sac.
