Well, I've got "girly" hands. The M200/M400 pens are a perfect size and weight for me. I passed on the M600 Pink and White LE when it came out because I got to hold one at a pen show and felt it was too heavy for me to use.
There's a woman in my local pen club who has small hands but likes hefty pens. And a guy who used to be in it who actually had a lot of thin, small, lightweight pens. It's just a matter of what feels right for each person and that's going to vary (and possibly also vary over time as well).
Ironically, the largest pen I own is the restored vintage Sheaffer Balance Oversize which had belonged to my husband's grandfather. It's large, but very light for its size.
